If you cannot access these forms please contact me at the business number via text. If scheduling for another person put their name, not yours. If you are redeeming a gift certificate/card or prepaid service text the business number to schedule your appointment.
I ask that you please reschedule or cancel at least 1 day before the beginning of your appointment or you may be charged a cancellation fee.
Although I understand unforeseen circumstances may arise, please cancel or reschedule appointments within 24hrs. If not done so, unless due to an emergency, the card on file will be charged a percentage of the total amount due for the session. In the event of a no show, the full amount due will be charged after 24hrs of the scheduled appointment is missed.
When arriving for your appointment please do not come to my front door. Park at the garage doors and text 409-351-5860 to notify of your arrival.
