If you cannot access these forms please contact me at the business number via text. If scheduling for another person put their name, not yours. Only book 90 minute sessions or longer after speaking with the therapist. If you book without doing so, you will receive a call or text if no reasoning is left in the notes section. If you are redeeming a gift certificate or prepaid service text the business number to schedule your appointment. I no longer sell gift certificates.
Booking deposits are non-refundable and required but if a reschedule is needed the deposit will carry over. Fully paid appointments that need to cancel without a reschedule can be refunded minus the deposit amount originally required with 24 hr notice of needing to cancel . Last minute cancellations require fee of 50% of the appointment price. This is due at the time of cancellation.
